Teresa L. Hazelbaker

Middletown...Teresa L.(Leasure) Hazelbaker, 59, resident of Middletown, passed away August 31, 2012 at St. Johns Medical Center.She was born in Anderson, April 2, 1953 the daughter of Don and Mildred Mosley Leasure.She had driven a school bus for many years for the Shenandoah School Corporation and was a 1971 graduate of Madison Heights High School. She had lived in Anderson before moving to Middletown 35 years ago.She had been a former member of the Order of the Eastern Star and lived her life to the fullest for her family especially her children and grandchild.Survivors include her husband of 40 years: Bill of Middletown, 2 children: Scott (Heather) Hazelbaker of Lapel and Tracey Hazelbaker (DeMarkus Hambie) of Middletown, her favorite grandaugher: Jasmine Hazelbaker of Lapel, grandog: Biscuit, mother: Mildred Leasure, 1 brother: Don Leasure and 1 sister: Jane Trueblood (John), 3 nephews: Ryan, Derek, and Shane Trueblood.A Celebration of Life Service will be held on Thursday evening at 6:00 PM at Ballard and Sons Funeral Home, 118 South 5th, Middletown with Rev. Marty Ballard of the Yorktown Church of the Nazarene officiating.Friends may call one hour before the services at the funeral home Thursday.Memorial contribution may be made to the Middletown EMS, Box 1, Middletown, IN.The Ballard's are honored to serve the Hazelbaker family in family ministry.
