Middletown...Tammy Jill Smith, 50 resident of Middletown, passed away May 17, 2011 at St. Johns Medical Center in Anderson.She was born in New Castle, October 11, 1960, the daughter of Ralph and Donna Faucett Smith and had worked as a caretaker for Phoenix Community Services.She was a member of the American Legions Womans Auxillary of Middletown.Survivors include her companion: Steve Phillips of Middletown, Children: Jason (Kyla) Smith of Shirley, Tiffaney Smith of Middletown and Clinton M. Phillips of Markleville, 7 grandchildren, 2 brothers: Randy Barkdull and Robin Barkdull of Ft. Wayne.Memorials services will be held on Saturday at 10:00 at Ballard and Sons Funeral Home, 118 South 5th, Middletown with Rev.
