Middletown....Tamela Kay (Hayes) Collis, 53, resident of Middletown, passed away December 20, 2016 at St. Vincent's Regional Hospital in Anderson.She was born in Muncie, April 30, 1963, the daughter of Clarence and Della Richards Hayes.She was a 1981 graduate of Muncie Central High School and worked as a supervisor at ICAP.She was a member of the NRA and White Union Christian Church.Survivors are her husband: John of Middletown, 2 siblings: Jim (Mary) Hayes of Middletown and Virginia (Terry) Moore of Redkey, several nieces and nephews, sister in law: Mary Ann Stewart of Anderson and special friends: Kathy (Bob) Pierce of Straughn and Shari (Tim) Neal of New Castle.A Celebration of her life will be held on Monday December 26th at 6:00 PM at White Union Christian Church, West of Sulphur Springs on US 36 with Pastor Gordon Barrett officiating.Friends may call at the church one hour before the services on Monday.Online Guestbook at: www.ballardandsons.comThe Ballard family is honored to celebrate the life of Tammy with family and friends.
